Who's Eligible?

All American College of Emergency Physicians members and/or legal Spouse under age 60 working 20+ hours/week and residents of the United States.

Rates &/or benefits may be changed on a class basis. No benefits are paid during the Waiting Period or on a retrospecitve basis. Billing Mode Options: Monthly, Quarterly, Semi-Annual or Annual.

Effective Date

Each Eligible Member or Employee or Employee or Eligible legal Spouse who gives us an Enrollment Form and pays the required premium for his or her coverage becomes an Insured Person on the later of:

  1. the Policy Effective Date; or
  2. the first day of the month on or next following the date we receive request; subject to the Deferred Effective Date provision.

Deferred Effective Date

If an Eligible Member or Employee or Eligible legal Spouse is to become:

  1. covered under the Policy; or
  2. covered for increased or extended benefits under the Policy;

and is not Actively-at-Work on that date, he or she will not be so covered until the first day of the month on or next following the date he or she is Actively-at-Work for 3 consecutive months.

Explanation of Terms

Disability Income Benefit Offsets

No coordination with other disability income plans (including any coordination with Worker's Compensation).

Exclusions:

The policy does not cover any loss resulting from:
  1. Intentionally self-inflicted Injury, suicide or attempted suicide, whether sane or insane;
  2. war or act of war, whether declared or undeclared;
  3. Injury sustained while in the Armed Forces of any country or international authority;
  4. Injury sustained while voluntarily taking drugs which federal law prohibits dispensing without a prescription, including sedatives, narcotics, barbiturates, amphetamines, or hallucinogens, unless the drug is taken as prescribed or administered by a licensed physician;
  5. Injury sustained while legally intoxicated.

Termination:

Non-payment of premium, cessation of work, reach 65 years of age, the date you cease to be a member of ACEP or no longer reside in the United States. Spousal coverage will terminate once they become legally separated or divorced from you or they no longer meet eligibility requirements.

THIS IS LIMITED TO ACCIDENT ONLY COVERAGE.

You have 30 days from your effective date of coverage to look over the program and discuss with your family and advisors. If you are not satisfied, you may return your Certificate within 30 days for a full premium refund, less any claims paid.
